Painful symptoms are caused by the edge of the nail pressing into the skin. Due to the injured skin, bacteria can enter the wound and cause inflammation. Possible symptoms include redness of the skin, increasing throbbing pain in the toe or finger (depending on the location), suppuration, or deformed nail growth. In this case, the edge of the nail is pushed into the nail bed by pressure, exerted for example by tight shoes. This causes the skin to become injured, irritated, and inflamed. Additionally, granulation tissue (proud flesh) can form due to the injured skin, which then grows over the nail. If cases of Unguis incarnatus have already occurred in the family, heredity (foot shape, for example splayfoot) can also be a possible cause. Toenails with different nail shapes can also grow in differently. There are normal nails, involuted nails, and pincer nails. However, the treatment for the different types of nails usually remains the same. In the case of involuted nails, pincer nails, and nails affected by fungus, the entire nail may need to be removed.        <\/p>\n\n<h2 class=\"wp-block-heading\">Conservative Treatment<\/h2>\n\n<p class=\"wp-block-paragraph\">There are various treatment methods provided by podiatry that can bring about improvement. Taping the nail can lead to improvement in the early stages. This involves sliding a strip of adhesive tape between the nail and the inflamed skin. This is attached in such a way that the skin is pulled outwards, giving the nail more space.    <\/p>\n\n<p class=\"wp-block-paragraph\">Brace treatment can be performed preventively by a podiatrist. This helps in cases of nails that are too rounded and therefore sit too deep in the nail fold. The treatment pulls the nail back up out of the side pockets. If the symptoms are chronic and do not disappear with this treatment, the ingrown nail must be treated medically with an antibiotic ointment.   <\/p>\n\n<p class=\"wp-block-paragraph\">If no improvement in symptoms is achieved through these conservative treatment methods, surgical treatment of the ingrown nail is indicated. There are three different options for this. <\/p>\n\n<h2 class=\"wp-block-heading\">Minimally Invasive Method (Non-Definitive)<\/h2>\n\n<p class=\"wp-block-paragraph\"><strong>Partial nail removal:<\/strong><br\/>In this procedure, part of the nail is cut out under local anesthesia without damaging the nail bed, and the damaged wound area or granulation tissue (proud flesh) is removed. As a result, the inflamed skin of the nail fold recovers immediately, and the inflammation and symptoms disappear within one to two days. The nail grows back to its original width over several months and usually causes no further symptoms.  <\/p>\n\n<p class=\"wp-block-paragraph\">The advantage of the minimally invasive method is that the treatment time is only half an hour, it can be performed even if there is inflammation, and healing takes only two to three days. This procedure can also be repeated several times and often leads to permanent freedom from symptoms. If partial nail removal fails repeatedly, I recommend the definitive minimally invasive method of phenolization.  <\/p>\n\n<p class=\"wp-block-paragraph\"><div class=\"rl-gallery-container class=\"wp-block-heading\">Minimally Invasive Method (Definitive)<\/h2>\n\n<p class=\"wp-block-paragraph\"><strong>Phenolization:<\/strong><br\/>Phenolization offers another option for nail treatment that is definitive<strong>.<\/strong> If possible, this procedure should be performed in a non-inflamed state. The procedure is performed under local anesthesia. After partial nail removal, the lateral nail root is definitively destroyed chemically with phenol. Finally, the wound is rinsed with NaCl (saline solution).   <\/p>\n\n<p class=\"wp-block-paragraph\">I only recommend this procedure after partial nail removal has failed, as there is a small risk of nail growth disorders occurring. For this reason, I recommend having the treatment performed in a non-inflamed state, because complications are more frequent in an inflamed state and the result is worse. The success rate for phenol treatment is 80-90%. I no longer use the method known as nail wedge resection, as it destroys healthy tissue of the nail edge. I perform a local flap surgery. This technique not only provides the best cosmetic result but also prevents the symptoms from recurring.   <\/p>\n\n<p class=\"wp-block-paragraph\">With this method, after disinfection, sterile draping, and the administration of local anesthesia, a piece of the toe or finger nail is removed along with the corresponding part of the nail bed. The resulting wound is covered with a flap from the side of the toe or finger. By narrowing the nail bed, the nail no longer grows into the nail fold.  <\/p>\n\n<p class=\"wp-block-paragraph\"><div class=\"rl-gallery-container rl-loading\" id=\"rl-gallery-container-4\" data-gallery_id=\"217\"> <div class=\"rl-gallery rl-basicgrid-gallery \" id=\"rl-gallery-4\" data-gallery_no=\"4\"> <div class=\"rl-gallery-item\"><a href=\"https:\/\/www.eingewachsenernagel.ch\/wp-content\/uploads\/2024\/07\/Zehe-gleich-nach-Operation.jpg\" title=\"\" data-rl_title=\"\" class=\"rl-gallery-link\" data-rl_caption=\"\" data-rel=\"lightbox-gallery-4\"><img loading=\"lazy\" decoding=\"async\" src=\"https:\/\/www.eingewachsenernagel.ch\/wp-content\/uploads\/2024\/07\/Zehe-gleich-nach-Operation-768x768.jpg\" width=\"768\" height=\"768\" alt=\"\" \/><span class=\"rl-gallery-caption\"><span class=\"rl-gallery-item-title\">Zehe gleich nach der Operation<\/span><\/span><\/a><\/div> <\/div> <\/div><br\/><\/p>\n\n<h2 class=\"wp-block-heading\">Aftercare<\/h2>\n\n<p class=\"wp-block-paragraph\">Aftercare for partial nail removal consists of rinsing the small open wound with a shower head for 7 to 10 days and applying antibiotic ointment. To protect the wound from any bacteria, it must be covered with a plaster for a few days. Incapacity for work is only necessary in selected cases (steel-toed boots or physical labor).  <\/p>\n\n<p class=\"wp-block-paragraph\">With phenolization, it should be noted that the aftercare is more extensive. You must not participate in sports or walk long distances for two weeks, and pain may occur for a few days after the procedure. For strenuous activities, up to about one week of incapacity for work is required. Here, too, the open wound must be rinsed with a shower head for 14 days and antibiotic ointment must be applied.   <\/p>\n\n<p class=\"wp-block-paragraph\">The aftercare period for definitive surgical nail narrowing lasts considerably longer. The wound must not get wet and must be covered with a plaster and bandage for at least 2 weeks. Furthermore, no weight should be put on the toe. Stitches are removed approximately 2 weeks later. Normal shoes can be worn again after three weeks. Sport is only possible again after 4-6 weeks. The duration of incapacity for work depends on the professional activity.      <\/p>\n\n<h2 class=\"wp-block-heading\">Costs<\/h2>\n\n<p class=\"wp-block-paragraph\">Costs are normally covered by mandatory basic health insurance for outpatient treatment.<\/p>\n\n<p class=\"wp-block-paragraph\">Costs always refer to one toe. For partial nail removal, costs between CHF 200 and CHF 260 should be expected. The cost of phenolization is approximately CHF 400. For definitive nail narrowing, the costs are between CHF 700 and CHF 900.
